Invisalign Consent Form

This form covers orthodontic treatment with clear aligners. The treatment is gradual and depends on your daily commitment to wearing the trays. We are partners in this — the result is shared work.

What we are agreeing to do

Plan and supervise an Invisalign course using digital impressions, a clinical plan, and a defined number of aligner trays. The plan is reviewed at each visit and adjusted where needed.

What we ask of you

• ·Wear each tray for the prescribed number of hours per day — typically 20 to 22 hours.
• ·Remove trays only to eat, drink anything other than water, and brush.
• ·Attend every check-in visit.
• ·Use the retainers we provide at the end of treatment, for life.

What you can expect

• ·Tightness and mild ache in the first 48 hours of each new tray.
• ·Speech adjustment in the first week.
• ·Visible progress between weeks four and eight.
• ·Refinement trays after the initial plan, in most cases.

What we do not promise

We do not promise that the result will hold without retainers. The teeth move because we move them; without retention, they move back. The lifelong retainer protocol is part of the treatment, not an afterthought.
